What to Bring to Your Appointment

Your first occupational therapy appointment in Hemingford typically lasts 45–60 minutes. Coming prepared helps us make the most of your time together.

What to Bring

Photo Identification: A government-issued photo ID

Insurance Card: Your current card if you plan to use insurance

Referral or Prescription: If required by your insurance plan or claim

Medication List: A current list of medications and dosages

Relevant Medical Records: Imaging reports, operative reports, physician notes, or other records related to your condition

Current Braces or Equipment: Any orthosis, splint, or adaptive equipment you currently use

What to Expect

Your first therapy appointment is a comprehensive occupational therapy evaluation - not simply a brief intake. TaLisa will review your medical history, discuss your concerns, assess your condition and functional limitations, and learn which daily activities are most important to you.

Together, you will establish meaningful goals and discuss initial treatment recommendations. When appropriate, treatment or education may begin during the first visit. By the end of your first visit, you'll have a personalized plan of care and a clear sense of what treatment will look like going forward.

Follow-up appointments are will focus on treatment, progress toward your goals, and adjustments to your individualized plan of care.

For Your Safety

If you recently had surgery, are taking prescription pain medication, or do not feel safe driving, please arrange for someone to bring you to and from your appointment.

Unless your provider has instructed otherwise, please eat a light meal and drink water before your visit. This can help reduce the risk of lightheadedness, particularly when postoperative dressings and removed or changed.

Please tell your therapist if you feel dizzy, lightheaded, nauseated or prone to fainting during treatment.
