How to Refer a Patient

We make the referral process simple. Here's how to get your patient on the schedule.

Send The Referral

Send the fax or order to (866) 877-2532

Please include:

  • • Patient demographics and contact information
    • Insurance information
    • Diagnosis and affected body region
    • Relevant precautions or post-operative protocol
    • Operative notes, imaging reports, or recent clinical documentation when available

We Contact the Patient

Once we receive the referral and confirm applicable insurance or authorization requirements, we will contact the patient directly to schedule their initial evaluation. Same-week appointments are often available.

Evaluation & Communication

The initial evaluation typically lasts 45–60 minutes. TaLisa will assess the patient's condition and functional limitations, establishes individualized goals, and develop a plan of care. Referring providers will receive evaluation findings, progress updates, and discharge recommendations.
